The following is a more detailed description of the data I capture online to support the Health Coaching practice. I do not share this information without your express permission.
If you submit a quick question via the Contact Me form, I gather your name, your email address, and the question that you ask. If the answer results in some back-and-forth, I will have the records of our exchange in the Hunt Gather Thrive email account.
All of the information you complete on the Discovery form will be sent to me via email, and I will capture it to facilitate our Discovery Call and form the foundation to support an ongoing coaching engagement, if you decide to proceed.
If you sign up for the Ask-A-Coach call, I gather your name and email address and the call topic. When you pay for the call, your payment information becomes subject to the square.com privacy policies (https://squareup.com/us/en/legal/general/privacy).
When you sign up for the Kickstart, square.com gathers your name and email address and payment details, so all of those are subject to their privacy policies (https://squareup.com/us/en/legal/general/privacy). I receive your name and email address from square.com, and retain them in order to send you the daily emails.
When you sign up for the 12-Week Healthy Habits Reset, I ask you to complete the online Intake Form. This form includes identifying information (e.g., Name, Age, email address, home address, phone number), your health goals, information about your current eating, exercise, sleep, work, stress, and energy, as well as questions about your medical history.
You may also be asked to provide information to track your progress in areas such as body composition or physical performance, your weekly check-ins, and the actions and agreements reached during coaching calls. When you pay for the program, your payment information becomes subject to the square.com privacy policies (https://squareup.com/us/en/legal/general/privacy).
I take the protection of your personal data very seriously. I receive the information from your online form submittals via email, and will maintain it in password-protected files on my password-protected computer in my office.
Please note that data transmitted via the internet may be subject to security breaches. Complete protection of your data from third-party access is not possible.
I do not share your information. If you are signing up for a service that requires a payment, the information used to make the payment is captured by our payment vendor and is subject to their privacy policies. I do not have access to your payment information at any time.
If, at any time, you wish to review or revoke the personal information I’m maintaining on file for you, please send an email to kelly@huntgatherthrive.com, and I will provide you a copy of your files, revise, or delete your data, as requested.
Many data processing operations are only possible with your express consent. You may revoke your consent at any time with future effect. An informal email making this request is sufficient. Please note that revoking consent doesn’t undo any use or processing of your data performed before I receive your request.
If there has been a breach of data protection legislation, the person affected may file a complaint with the competent regulatory authorities.
You have the right to have data which I process based on your consent or in fulfillment of a contract automatically delivered to yourself or to a third party in a standard, machine-readable format. If you require the direct transfer of data to another responsible party, this will only be done to the extent technically feasible.
As permitted by law, you have the right to be provided at any time with information free of charge about any of your personal data that is stored as well as its origin, the recipient and the purpose for which it has been processed. I hereby expressly prohibit the use of contact data published in the context of my website legal notice requirements with regard to sending promotional and informational materials not expressly requested.
